All names should be listed as they appear in official CFU records
Use MM/DD/YYY for the date of birth format
Shirt size should be one of the following choices: Youth (S, M, L, XL) or Adult (S, M, L, XL, 2XL, 3XL, 4XL)

Enter the number of each shirt size required for non-performing travelers

These numbers should add up to the total number of non-performers traveling with your group entered above
